Documentation of Receipt: Center Operational Policies

By signing below, you are acknowledging you have read and fully understand the guidelines set forth in the Children's Unlimited Academy Handbook of Operational Policies. I have a copy of this handbook for my personal reference.

Documentation of Receipt: Prevention of Shaken Baby Syndrome and Abusive Head Trauma Policy

Parent or guardian acknowledges that I have read and received a copy of the facility's Shaken Baby Syndrome/Abusive Head Trauma Policy.

Notification of Smoking & Tobacco Restriction:

Children's Unlimited Academy childcare center is a smoke and tobacco free environment. During operating hours, smoking and the use of any product containing, made, or derived from tobacco is not permitted on the premises, in vehicles used to transport children, or during off-premises activities. Parents will be notified, in writing, of the smoking and tobacco policy. Signs will be posted at the facility entrance and in vehicles used to transport children.

Documentation of Discussion: Parent Involvement & Participation

Your signature initials on the section of the form means that Children's Unlimited Academy has a plan that is discussed with parents on or before the child's first day of attendance concerning items listed below:

• A procedure for registering a child for child care that involves both parents when possible & encourage a visit to the center by the child and the child's parents before the child begins attending the center.
• Opportunities for caregiving staff to meet with parents on a regular basis to discuss their child's needs and progress and to exchange information about the program
• Activities that provide parents opportunities to participate in the centers program on an individual basis and as a group
• A procedure for parents who needs information or have complaints about the child care program.

Documentation of Receipt: Discipline Policy

Parent or guardian of acknowledges that I have read and received a copy of the facilities Discipline & Behavior Management Policy and that the facility director/operator (or other designated staff member) has discussed the facilities discipline and behavior management policy with me.

Documentation of Receipt: Safe Sleep Policy

Parent or guardian of acknowledges that has received a copy of the facility's infant toddler safe sleep policy. I have read the policy and discussed it with the facility director/operator or other designated staff member.

Documentation of Receipt: Summary of Child Care Laws

I have reviewed a posted copy of the Summary of N.C. Child Care Law for North Carolina in the center. I also have access an electronic copy to view as needed.

I have read and fully understand the guidelines and procedures set forth in the summary child care law.
